The turning-65 checklist

Six months out, three months out, and the month itself — what to do and when, so nothing lands as a permanent penalty.

Turning 65 is the only insurance deadline most people meet once, which is precisely why it goes wrong. The rules are not complicated, but they are unforgiving: several of the mistakes available to you cost a surcharge for life.

Here is the sequence we walk clients through, starting six months before the birthday.

Six months out: decide whether you are delaying

If you or your spouse are actively working for a large employer and covered by its plan, you can usually delay Part B without penalty. If you are retired, on COBRA, or on retiree coverage, you cannot — those do not protect you, and that single distinction causes more permanent penalties than anything else in Medicare.

If you contribute to a health savings account, this is also the month to stop. Part A can be backdated up to six months when you enrol, and contributions made during those retroactive months become excess contributions.

Three months out: your window opens

The initial enrolment period runs seven months: the three before your birthday month, the month itself, and the three after. Enrolling in the first three means cover starts on the first of your birthday month. Waiting until after means a delay before anything is active.

This is also the point to gather the two lists that drive every plan comparison: your doctors, and your medication with doses. Without them, any recommendation is guesswork dressed up as advice.

The month itself: choose a structure, not a brand

The real decision is Original Medicare with a Supplement and a drug plan, or a Medicare Advantage plan. That is a choice about how you want to access care and where you want the risk — networks and copays against premiums and freedom — and it is far more consequential than which carrier's name is on the card.

Whichever you choose, put a reminder in the calendar for the following autumn. Plans change every year, and the plan that fits this year is regularly not the one that fits next.
